For Juvenile Clients (Ages 17 and Under):

At Overcomers Counseling in Farmington, NM, we are committed to providing comprehensive support to our juvenile clients. If you are 17 years old or younger and wish to create a client account with us, please note that your parent or legal guardian will need to complete certain steps on your behalf.

Parental Involvement Required:

To ensure the well-being and best possible care for our young clients, we require parental involvement during the account creation process. Here's what you need to do:

1. Parental Consent: Your parent or legal guardian must provide their consent for you to create a client account with Overcomers Counseling. They will be required to sign a consent form, indicating their approval of your participation in our counseling services.
2. Application and Assessment: After obtaining parental consent, our team will provide your parent or legal guardian with a specific application and assessment form designed for juvenile clients. This form must be completed accurately and submitted as part of your account setup.
